Laser Enucleation of the Prostate
(a.k.a. HoLEP/ ThuLEP/ ThuFLEP) 

For decades, the standard surgery for enlarged prostate has been Transurethral Resection of the Prostate (TURP), commonly known as “prostate shaving surgery.”

While TURP remains effective, advances in medical technology have led to newer techniques that may offer improved outcomes for selected patients. One of the most advanced options available today is Laser Enucleation of the Prostate.

Importantly, only a limited number of urologists in Malaysia currently offer prostate enucleation surgery /HoLEP due to the specialized expertise, dedicated equipment, and steep learning curve required for the procedure.

What is this surgery?

Prostate enucleation (eg. HoLEP) is a minimally invasive laser surgery used to treat enlarged prostate. It uses a high-powered laser to remove the entire obstructing prostate tissue more completely, similar to removing the flesh of an orange while preserving the peel.

Safety of the surgery

Patients should understand that HoLEP, like TURP, commonly results in retrograde ejaculation (“dry orgasm”), where semen flows backward into the bladder instead of coming out.

This does not affect sexual pleasure but can affect fertility.

Temporary urinary urgency, mild leakage, or burning during urination may also occur during recovery, although most improve over time.

Serious complications are uncommon when performed by experienced surgeons.

HoLEP is a highly specialized surgery with a significant learning curve.

Successful outcomes depend greatly on surgeon experience and familiarity with advanced laser techniques.
